Please print out the form, fill out, and then mail the form and check back to CyRide at the address below.

Make check for the exact amount payable to CyRide. Please do not mail cash.

Please note, ISU Students with a current ISUCard do not need a bus pass or tickets. Just show your student ISUCard and ride for free with your payment of tuition which includes fees to the ISU Student Government who funds CyRide.
